So, I just wrote a thread about how ridiculous it is to find certain uniques in Diablo. Afterwards I went and did a few more runs and low and behold, there it was.

Here are some tips to finding it, for anyone who's willing to put forth the effort.

For those of you who don't know, the Helm of Sprits is the only item in the (multiplayer) game that offers leech and isn't a weapon. It provides no other mods, but is totally worth it. Try it--especially with a weapon of Haste--and you'll see why. You won't use nearly as many potions if any.

There are 8 unique monsters that can drop the Helm of Sprits and they are all found on dungeon level 2 on Nightmare difficulty:

The Butcher
Rotfeast the Hungry - Zombie
Soulpus - Zombie
Pukerat the Unclean - Fallen One (w/Spear)
Bladeskin the Slasher - Fallen One (w/Sword)
Boneripper - Skeleton (w/Axes)
Bonehead Keenaxe - Corpse Axe
Shadowbite - Scavenger

All these monsters have a mlvl lower than 7 to prevent the Overlord's Helm from dropping instead. There is only one other unique monster on this level: Deadeye (Skeleton Archer), who can't drop the Helm of Sprits.

Every run, you'll notice that there are usually 4 kinds of enemy in dlvl 2 besides the Butcher (occasionally they'll be 3 or 5). Note the kind of enemy and it will tell you if you can find a special monster. For example, if you see a Zombie, then you know that Rotfeast and Soulpus are both down there for the killing.

This means that you can find anywhere from 1 to 6 unique monsters on dlvl 2 (the average is right around 3).

From a statistical standpoint, The Butcher has a 1 in 475 chance to drop Helm of Sprits. The other monsters have a 1 in 437.5 chance. Since there are usually three chances per run (one always being the Butcher), you have the probability of 1 in 150 chance to find Helm of Sprits per run through dlvl 2 on Nightmare.

Excellent! I've just started to play D1HellFire, I can't wait to lvl enough to make runs... what level do you have to be to make it a clean sweep w/o incurring too much damage?
